Inquiry & Important Information

Dear Potential Mater Dei School Family,

We appreciate your interest in Mater Dei Catholic School.  All requests for admission must be initiated with a completed and submitted inquiry form.  Once the form is received, and it is determined that space is available, you will be contacted for additional information, and the admissions process will be explained. If space is unavailable, you will automatically be placed on the waitlist, and a confirmation will be emailed to you.  Please know that if you are placed on a waitlist for a specific school year, and we do not have room, you must fill out a new inquiry the following year if you are still interested in enrolling.  Waitlists do not carry over from year to year.  Due to the high number of requests for tours at our school, campus tours will only be scheduled in small groups, individually, and only through appointment. It is difficult to accommodate a drop-in tour.

 

Process and Admissions Requirements for all Students


Our Admission and Registration process is easy, and most requested information can be submitted online. You will be provided with the required forms as the process progresses. Some of the documents needed for all grades include:

Birth Certificate
Baptismal & Eucharistic certificates if applicable
Health and Immunization Records on state forms DH3040 and DH680   
Academic records, including current and prior year report cards and standardized test scores

Requirements by Grade


Pre-school Three-year-old Program Requirements:

Must be three (3) years old on or before September 1.
Must be thoroughly toilet-trained
Your child may start after turning three; however, the child would be required to attend the Three-year Preschool the second year before beginning our VPK four-year-old program.


Pre-Kindergarten Four-Year-Old Program Requirements:

Must be four (4) years old on or before September 1.
Must be thoroughly toilet-trained
In-classroom screening by PK Teacher

Kindergarten Requirements:

Must be five (5) years old by September 1.
Report card from any previous educational program
A letter of recommendation from the administrator of a previous educational program
In-classroom screening by our Kindergarten Teacher


First Grade through Eighth Grade Requirements:

Report cards from the last two completed years, including the most recent report card
Standardized test scores from the last two years, as applicable
Recommendations from the previous school’s administrator, math and reading teachers
Interview with the school administrator and a shadow day in the applicable grade


Before a student can attend class:

A new or updated physical is required for EC, Kindergarten, and 7th Grade, and all new students. Form DH 3040 (ORIGINAL physical form) and form DH 680 (ORIGINAL immunization form).
All children entering Kindergarten must have had all three Hepatitis B shots and a second Varicella vaccine. If the fourth dose of polio vaccine is administered prior to the fourth birthday, the fifth dose is required for kindergarten entry.

The Diocese of St. Petersburg does not accept Religious Exemptions. Please ask your family physician if you have any concerns or questions regarding these immunization requirements. Only Medical Exemptions will be considered when a Florida doctor signs and includes them on the proper state forms (DH680). An annual update will be required along with an immunization plan. Your doctor MUST upload the immunization record to the Florida Shots website.
